Effective January 1, 2006 

Bostwick Laboratories® accepts all insurance. We bill the patient for the appropriate co-pay or deductible. It is the responsibility of the referring office to provide supplemental insurance information when appropriate. Your patient will not be responsible for the difference between Bostwick Laboratories'® list price and the payor's allowable amount, except when services are denied under provisions of the patient's behalf when appropriate. Every effort will be made to appeal denials on the patient's behalf when appropriate. For patients who self-pay, the patient is responsible for the billed amount. Patients may contact our billing office to establish a payment plan, if appropriate.
